Once you are a patient, your health information is protected by federal law (HIPAA) and handled as described in our Notice of Privacy Practices.
Information you give us
Callback requests. If you use the “Request a Callback” form, we collect your name, your phone number, and the office closest to you. The form sends these details by email to our scheduling staff. We use them only to call you back about scheduling. Please don’t include medical details in the form. We’ll ask what we need when we call.
Phone calls and faxes. When you call or fax us, the information you share is handled as part of your care and is covered by our Notice of Privacy Practices.
Information collected automatically
Website statistics. We use Google Analytics to understand how people use this website, for example which pages are visited most and how visitors find us. Google Analytics uses cookies and similar technology to collect information such as the pages you view, how you arrived at the site, your general location (city or region), and the type of device and browser you use. We have turned off Google’s advertising features (Google signals and ad personalization) for this website. We don’t use this information for advertising, and we don’t send the contents of the callback form to Google. You can read how Google uses this information.
Server records. Like most websites, our hosting provider automatically records basic technical information when you visit, such as your IP address, browser type, and the pages requested. These records are used to run and secure the website.
Spam protection. To stop automated abuse of the callback form, our server briefly keeps scrambled (hashed) copies of the IP address and phone number used to submit the form, so it can limit repeated requests. These are deleted automatically within about a day.
Cookies
The only cookies this website sets are the Google Analytics cookies described above. We do not use advertising or social media tracking cookies. You can block or delete cookies in your browser settings, or install the Google Analytics opt-out browser add-on. The website works the same either way.
How we share information
We do not sell or rent your personal information. We share it only:
- with service providers that help us run this website and deliver email, such as our web host, our email provider, and Google Analytics, who may use it only to provide those services to us;
- when required by law, or to protect the rights, safety, and security of our patients, staff, or others.
Links to other websites
This website links to other sites, such as Google Maps for directions. Those sites have their own privacy policies, and we are not responsible for how they handle information.
Children
This website is not directed to children under 13, and we do not knowingly collect information from them through the website. A parent or guardian can call us to schedule a study for a child.
Security
This website uses an encrypted (https) connection. No method of sending information over the internet is completely secure, so please call us at 801-871-5228 if you prefer not to use the online form.
